I'm heading to Medellin in a few weeks, and am completely new to the p4p scene. And with my inexperience, I feel like it is awkward enough going to a casa, let alone going to one with sole reliance on Google translate.

So are places like Ellas Erotic, The Garden, Golden House etc used to foreign customers showing up at the door or messaging on Telegram? It would be nice if the girls spoke a little English also, but I know that's expecting too much.

On another note, better to book in Laureles or El Poblado as a first-timer? I know the former is closer to casas, and the latter is more english friendly.

They are used to you showing up but you can text them first. WhatsApp is generally better for appointments, TG is generally for advertising. Some don't mind which you use, but WhatsApp is always good so just stick to that.

You nailed the difference between the two. I'd spend a few days in either depending on the length of your trip.
